Readiness Assessment Intro

This assessment examines the organizational conditions that foster stability—specifically psychological safety, predictability, respectful accountability, employee voice, cultural humility, and trust repair. It is designed to evaluate readiness for trauma-informed transitions, not to diagnose individuals or ask for personal trauma disclosures.

The assessment consists of 10 items rated on a 5-point scale (1-5). Total scores range from 10-50, mapped across three readiness bands to guide your strategic focus.

Scoring Bands & Readiness Levels

Add your ratings for all 10 questions to get a total score from 10–50. Your total identifies the organizational conditions currently present in your leadership environment.

40–50 – Trauma-Informed Leadership Foundation

The organization appears to have several consistent practices related to safety, trust, communication, employee voice, and accountability. Leadership should examine whether these conditions are experienced consistently across departments, shifts, and supervisory levels.

26–39 – Inconsistent Trauma-Informed Conditions

Trauma-informed practices may depend on the individual leader, department, situation, or level of organizational pressure. A structured review can identify where communication, psychological safety, support, or follow-through becomes less predictable.

10–25 – Priority Leadership Intervention

Employees may experience limited psychological safety, unclear communication, primarily corrective management, or unresolved trust concerns. Immediate attention should focus on reporting safety, manager behavior, critical-incident response, moral distress, and trust repair.

Next Steps & Guidance

Use your total score to identify your organization's specific path forward. The bands below provide initial direction for trauma-informed leadership transitions.

Score 40–50: Optimizing the Foundation

Focus on scalability and sustainability. Document existing informal safety practices into formal policies and mentor emerging leaders to maintain culture during growth or turnover.

Score 26–39: Structural Stabilization

Identify where consistency breaks down. Focus on middle-management training and creating predictable communication channels to close the gap between leadership intent and staff experience.

Score 10–25: Critical Safety Restoration

Priority should be on high-impact safety interventions. Immediate steps include launching trust repair processes, clarifying boundaries, and addressing systemic stressors causing moral distress.
